Dr. César De La Rosa’s virtual consultation form is designed to gather the basic necessary information commonly requested in a personal consultation. However, it in no way substitutes for the in-person consultation, which is what truly provides the most important details (such as those obtained in the physical examination) to reach a more accurate diagnosis. Therefore, the medical opinions generated from this virtual consultation only represent an approximate reference that must be verified in the in-person medical evaluation.

1. FILLING OUT THE FORM

1.1 The virtual consultation form must be filled out completely. The information requested is important to gain a better idea of your physical conditions for undergoing the desired procedure.

1.2 The information you provide is taken as true and is strictly confidential. It is taken into account to obtain the best medical or surgical opinion. If any data is false or omitted, it may affect the medical course of action initially provided to you.

1.3 The photographs requested are strictly confidential and may only be used for scientific or demonstrative purposes. They are deleted immediately from the database once the consultation is finalized.

1.4 Your supplied personal data will not be disclosed under any circumstances.

2. VIRTUAL CONSULTATION

2.1 In this consultation, you may ask any questions about the desired procedure, which will be answered based on the information you provided and considering its limitations. As it is not an in-person consultation, it needs to be supplemented later through a direct physical examination.

2.2 After you submit this form fully completed, you will receive a response via email or text message confirming the date and time the virtual consultation will take place.

2.3 The online consultation will be conducted via video call using the WhatsApp platform. Dr. César De La Rosa is not responsible for internet connection failures that may occur during the consultation. 2.4 The opinions, surgical medical plans, and eventual costs of the procedure resulting from this consultation are subject to change once the in-person physical evaluation (mandatory) with Dr. César De La Rosa is performed.

2.5 For a better assessment through the virtual consultation, you may be asked for additional information or new image projections to complement the information initially submitted by you.

2.6 The response time is within a period of 48 hours once the fully completed form is received. However, it may be extended to a longer response period due to reasons beyond the control of Dr. César De La Rosa.

2.7 If you do not receive a response regarding your consultation within a period greater than 48 hours, please contact us again.
