The virtual consultation form of Dr. César De La Rosa is designed to collect the necessary basic information that   is commonly requested in a personal consultation, however it does not replace in any way the face-to-face consultation that is the one that really provides the most important details (such as those obtained in the physical examination) to obtain a more accurate diagnosis. Therefore, the medical opinions generated from this virtual consultation only represent an approximate reference that must be verified in the face-to-face medical evaluation.

1. FILLING OUT THE FORM

1.1The virtual consultation form must be filled out completely, all the information requested is important to have a better idea of the physical conditions that you have to undergo the desired procedure.
1.2The information provided by you is taken for granted and is strictly confidential, it is taken into account to obtain the best medical or surgical criteria. If any information is false or obviated, it may have an impact on the medical conduct that was initially provided.
1.3The requested photographs are strictly confidential and can only be used for scientific or demonstrative purposes. They are deleted from the database of this query 48 hours after being answered.
1.4For no personal data supplied by you are disclosed.

2. VIRTUAL CONSULTATION

2.1In this consultation you can ask any question about the procedure you want, which will be answered based on the information provided taking into account its limitations, at  not being a consultation made from Personally there are important data that can only be obtained by direct physical examination.
2.2After you send this form completely filled out, you will receive a response via  email or text message.
23Opinions, surgical medical approaches and possible costs of the procedure  product of this consultation  are subject to change once the complementary (mandatory) physical evaluation is carried out in person with Dr. Cesar De La Rosa.
2.4For a better assessment through the virtual consultation, additional information or new image projections may be requested to complement the information initially provided by you.
2.5The response time is within a period of 48 hours once the completely filled out form is received. However, it can be extended to a longer response period for reasons beyond the control of Dr. César De La Rosa.
2.6If you do not receive a response to your query in a period greater than  los 48 hrs. please contact us again.
